Diseases.
Key Responsibilities
Interview clients to assess their needs related to personal and
family adjustments, finances, and other areas
Gather and evaluate information on clients' medical, psychological,
and social factors to determine appropriate interventions
Refer clients to community resources and maintain records and
reports on client services and case status
Required Qualifications
Bachelor's degree in a health-related field such as social work,
counseling, sociology, or psychology
Equivalent experience in case management may be considered for
candidates with degrees in other fields
No specific certifications are required for this position
No specific work experience is required for this position
A driver's license is not required for this position
