The Nurse Practitioner (NP) or Physician Assistant (PA) reports to the APP Lead and Practice Manager, playing a key role in strategic decision-making, clinical judgment, and evidence-based analysis to support the organizations mission. This role requires strong organizational skills and a commitment to delivering high-quality care while aligning with core values: Trust Worthy, Empathetic, Committed, Humble, Creative, and Community-Minded.As an essential member of the primary care team, you will: Conduct annual health assessments to ensure comprehensive patient care. Provide cost-effective, high-quality care to assigned Family Members. Collaborate across a multi-disciplinary team to develop, implement, and refine home health.Job Responsibilities

  • In partnership with the clinical team of CMP, provide medical highest quality medical care for the practice's patients.
  • In partnership with the Medical Director and Management, assist and support clinical personnel (e.g., clinical nurses, allied health professionals, EMR, telemedicine operations).
  • Provide consultations and perform diagnostic and therapeutic procedures in their clinical discipline within their practice scope, consulting when appropriate the supervising physicians for assistance with both diagnosis and treatment.
  • Works collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care.
  • Commit to the practice of value-based care which emphasizes care in the appropriate setting to improve and lower costs, address and close gaps in care and drive high patient satisfaction.
  • Demonstrate proficiency and support use of the practice management system and electronic medical record (EMR).Required Qualifications
    • FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA
    • Unrestricted NP or PA license in the state of NY.
    • Current DEA licensure / Prescriptive Authority post-hire
    • 2+ years of clinical experience as an NP/PA in the primary care or home care setting
    • Working knowledge of chronic care management and various medication options
    • Experience in Family Medicine, Internal Medicine, Gerontology, Palliative CareBenefits Offered

Strong communication skills are essential, as you will interact with customers, vendors, management, and co-workers both in person and via various devices. At times, you may need to engage with individuals who are agitated, requiring professionalism and composure.Key responsibilities include: Regular use of telephone and email for effective communication. Ability to sit or stand for extended periods as needed. Receiving and processing information, including reviewing and preparing documents. Occasional lifting of up to 50 lbs. Manual dexterity for handling office equipment such as computers, calculators, copiers, and fax machines. Strong reasoning skills to interpret management reports, memos, and other business-related documents.This position requires adaptability, problem-solving abilities, and a detail-oriented approach to ensure smooth operations and effective communication across all levels of the organization.
