Great Lakes Physician Practice (GLPP) is hiring a full-time
Nurse Practitioner to support the Nephrology department. This is a
growing nephrology practice. This is primarily an outpatient role
with occasional inpatient duties. The schedule is Monday-Friday

(APP) is a Physician Assistant or Certified Registered Nurse
Practitioner and a member of the care delivery team. The Staff APP
is responsible to set the standards for the level and quality of
care. The Staff APP has responsibility, authority and
accountability for the provision of care through supervision by and
collaboration with a physician. The Staff APP manages and provides
patient care activities for a group of patients and their families
through the application of independent judgment, communication and
collaboration with all team members. The role of the Staff APP
encompasses the beginnings of leadership, partnership,
collaboration and supervision. The Staff APP establishes and
maintains collaborative relationships with physicians, other health
care providers, patients and their families, to achieve desired
patient outcomes throughout the continuum of care. The Staff APP
begins to demonstrate a commitment to the community and to their
healthcare profession.Senior Level:The Senior Professional Advanced
Practice Provider (APP) is a Physician Assistant or Certified

their healthcare profession.The successful candidate will be placed
according to years and months of experience for employment
purposes.Demonstrate the knowledge and skills necessary to provide
care and/or interact appropriately to the patients served as
specified belowDemonstrate knowledge of the principles of growth
and development over the life spanPossess the ability to assess
data reflective of the patient's status and interpret the
appropriate information needed to identify each patient's
requirements relative to his/her age-specific needsProvide the care
needs as described in the department policy and proceduresSpecial
Skills and Abilities Required:Ability to establish and maintain

physicians, non-physician providers, ancillary and support staff,
other departments, and patients/familiesAbility to work
productively and effectively within a complex environmentHandle
multiple/changing priorities and specialized equipmentCritical
thinking, analytical and problem-solving abilities required as
related to various aspects of patient careGood clinical
judgmentCritical thinking skills necessary to exercise and lead

and visual manual dexterityPhysical stamina for frequent walking,
standing, lifting and positioning of patientsThis position is
dependent upon successful attainment and maintenance of hospital
privileges, if applicable to the Business Unit practice.Licensure,
Certifications, and Clearances:Current NY state RN license and NP
certification is required. If practicing in an inpatient setting
one of the following certifications most appropriate to the patient
population being cared for: Advanced Cardiac Life Support
Certification Pediatric Advanced Life Support (PALS) Advanced
Trauma Life Support (ATLS) Neonatal Advanced Life Support (NALS) If
practicing in an outpatient or other non-inpatient setting, one of
the certifications listed above OR achievement of a department /
business unit approved certification which is relevant and value
added to the practice setting. National certification, eligible for
licensure by practicing state board of nursing, eligible for
certification as a NP by practicing state board of nursing. CPR
required based on AHA standards that include both a didactic and
skills demonstration component within 30 days of hire.Advanced
Cardiac Life Support (ACLS) OR Basic Life Support (BLS) OR
Cardiopulmonary Resuscitation (CPR)Nurse Practitioner
(NY)Registered Nurse (RN)Senior Level:Minimum 3 years of experience
or Completion of a post-graduate residencyThe successful completion
of an approved Nurse Practitioner or a graduate of a physician
assistant program accredited by the Accreditation Review Commission
on Education for the physician assistant (ARC-PA) program is
requiredBSN, MSN preferred for CRNP, BS or MS preferred for
PACompletion of National CertificationMust achieve a
Solid/Strong/Good performance rating or higher on annual
performance evaluation for consideration and to maintain status
annuallyMust have department chair approval for promotionACLS or
ITLS or NALS or PALS or ACLS or other certifications listed must
meet AHA standards that include both a didactic and skills
demonstration component within 30 days of hire.OAPP: If practicing
in an outpatient or other non-inpatient setting, APPs can use one
of the certifications above or use a business unit approved
certification which is relevant and value added to the practice
