About the RoleWe are seeking a skilled Pediatric RN Case Manager to join our team at Rochester Regional Health. As a key member of our Home Care team, you will be responsible for delivering high-quality, patient-centered care to children and families in the comfort of their own homes.Key Responsibilities

• Develop and implement comprehensive plans of care for pediatric patients, in collaboration with the attending physician and other care team members.
• Manage and coordinate patient care, including clinically complex cases, to ensure efficient and effective delivery of services and community supports.
• Communicate effectively with patients, families, and care team members to ensure seamless care coordination and timely updates on patient status and service needs.
• Document all patient care and coordinating activities in accordance with agency standards and corporate policies.Requirements
  • AS in Nursing (Required)
  • Current RN licensure in New York State
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skillsWhat We Offer
